Instructions:

• This year’s dinner is TO GO ONLY.

• Dinners are $20 each. Dinner includes — Korean BBQ Beef, Broccoli, Salad, Rice and Dessert.

• Please “Register” to reserve a time slot to pick up the number of dinners you are purchasing.

• LIMITED QUANTITIES PER TIME SLOT! ORDER EARLY!

• PAYMENT OPTIONS: (1) Check payable to “Troop 611” given to any Troop 611 Boy Scout. Please make sure to include your email or phone number on the check. (2) Check payable to “Troop 611” and send to the church office at San Jose Buddhist Church Betsuin, 640 N. 5th Street, San Jose, CA 95112; Attn: Boy Scout Family Dinner. Please make sure to include your email or phone number on the check. OR (3) By PayPal using this link: https://www.paypal.com/paypalme/BSTroop611Treasurer. Please make sure to include “Boy Scout Family Dinner”, your name and phone number in the “Add a note” section.

• Last day to reserve dinner(s) is Friday, March 5. Payment must be received no later than March 9.

• You will receive an email confirmation when you reserve your dinner(s) and another email once your payment has been received. Dinner(s) will not be able to be distributed until your payment is received.

• For every dinner purchased, a ticket will be entered into the General raffle. Additional raffle tickets will be available for purchase at pick up.

• Dinner pick up will be at San Jose Buddhist Church Betsuin.
